Physician Education
The Cognitive Neurology and Alzheimers Disease Center (CNADC)
- Northwestern University Feinberg School of Medicine has established
a Primary Care Provider Program for the purpose of assisting physicians
in the evaluation, treatment and care of cognitively impaired patients
and caregiving families. Alzheimers disease (AD) is a significant
medical and social problem for 4 million Americans, their caregivers,
families, and friends. With the increased number of new treatments
available, one of our goals is to help patients receive the earliest
possible diagnosis. In the absence of a convenient and reliable
laboratory test for AD, the clinical assessment is still the cornerstone
of the diagnostic approach.
The CNADC has compiled dementia diagnostic guidelines and is happy
to provide a concise informational packet on dementia diagnosis
and treatment, in addition to Illinois area resources. As a Primary
Provider, you will receive updated material on a regular basis which
we hope will be of benefit to you in your practice. In order to
become a member of the CNADC Primary Provider Network, please print
out and complete the questionnaire
(in PDF format) and fax it to the attention of Darby Morhardt at
312-908-8789.
NOTE: Adobe Reader is required to download and view the questionnaire.
If you do not have Reader, it can be downloaded for free at Adobe
Reader.
You will receive the initial Primary Provider packet and will be
placed on our mailing list to receive regularly updated materials
and information. Please feel free to contact Darby
Morhardt with any questions. We value your impressions regarding
this program. Thank you for helping us in our efforts to improve
the care and treatment for persons with memory loss and their families.
