To Make An Appointment

Please call the Neurobehavior and Memory Health Services Clinic

Jackie Betti and Anthony Nowaske will be happy to help you schedule an appointment. Please call 312-695-9627. At the time of scheduling your initial appointment, you will be asked to obtain all pertinent previous medical records for the patient before the appointment date. Once the appointment is scheduled, we will send out a patient information packet to provide you with more information about what to expect as well as other information as to what the patient should bring to the appointment.

If our office does not have the patient's previous medical records before the scheduled appointment, it is likely that we will have to cancel the appointment.

Information on Obtaining Previous Medical Records

Due to the HIPAA laws, many medical offices and facilities require a written request before they will release a patient's medical history. You may use the following medical release to send to physicians or facilities that have your pertinent records.

If you are unsure about what records may be useful, typically they may include:

  • Medical Notes from neurologists, psychiatrists or family physicians
  • Previous neuropsychological testing reports
  • Laboratory reports such as:
    • EEG
    • Blood tests
    • Sleep study
    • MRI film of the brain
    • CT film of the brain
    • PET film of the brain
    • SPECT film of the brain
